住院医生一览

概述

The PGY2 Ambulatory Care Residency program through 奥尔巴尼药学与健康科学学院 (ACPHS) and 社区护理医生, (CCP) is designed to transition PGY1 resident graduates from generalized practice to specialize in chronic disease state management, 药物治疗管理, and 预防保健 within the primary care setting.

The program provides multidisciplinary patient centered experiences caring for diverse populations across the lifespan. Along with opportunities to hone direct patient care skills, completion of this program will produce residents who possess competencies that position them for attainment of board certification in ambulatory care pharmacy (BCACP). 

The ACPHS and CCP PGY2 Ambulatory Care residency program shares the core values of the 大学 and medical group, which focus on a sense of belonging and inclusivity. The core values aim to establish a welcoming and inclusive environment and consider diversity as a source of innovation, 浓缩, 和力量.

项目活动

直接病人护理

The resident will work alongside primary care practitioners and other members of the healthcare team to enhance patient care. The resident will have the opportunity to improve patient care by providing disease state management, 药物治疗管理, 预防保健, 病人和照顾者教育, and have the ability to address financial barriers to medication use. The resident will also be a part of other programs identified throughout the residency program. The resident will be involved in developing and participating in population health projects to assess disease prevention and quality of care. 

专业发展

The resident will participate in activities that advance pharmacy practice within primary care. The resident will be a member of the Pharmacy Services department at 社区护理医生 and assist in growing the pharmacy program. The resident will learn first-hand about the payment structures that currently sustain the Pharmacy Services department within the primary care setting and work with the team to identify new and innovative ways to expand the program. 

教学

The resident will participate in didactic teaching in large and small classroom settings, as well as experiential teaching of rotation students through ACPHS. If not previously completed, the resident may also complete a 教学 and Learning Program at ACPHS. Academic detailing will also be provided by the resident for clinicians within 社区护理医生.

基于实践的研究

The resident will be involved in research and other scholarly projects throughout the year.  The resident will complete a research project during the year that is expected to be publishable by nature and eligible for submission at a national conference.

居民资格

Before applying for the PGY2 Ambulatory Care residency, applicants must:

• Be a graduate of an accredited school or 大学 of pharmacy
• Have completed or expect to complete a PGY1 residency program
• 有资格获得纽约驾照. The resident must complete licensure within two months of beginning the residency.

重点是人际交往能力, 较强的沟通能力, and a desire to provide patient centered care in the primary care setting. The candidate should be self-motivated and display a commitment to professional development.

Interested applicants should submit a letter of intent, 简历, and two letters of recommendations to PhORCAS by the second Monday in January.
